Health Forms and Information for 2026
All forms will be submitted via your CampInTouch account or through your Campanion account
ALL Campers must complete the following Health Forms & submit via CampInTouch:
- Health History (found on CampInTouch)
- Immunization Form (use our form or one from your pediatrician)
- Over The Counter Medical Authorization
If your camper has medication / medical needs at camp, please complete the applicable forms below and submit to CampInTouch to the "Medication / Extra Medical Forms:
- Medical Authorization (for ANY medication administered at camp)
- Food Allergy Action Plan
- Asthma Action Plan
- Diabetic Action Plan
- Seizure Action Plan
